UNIVERSITY OF NEVADA Waiver for Participation
I am a student enrolled at The University of Nevada, Reno, a member institution of the Nevada System of Higher Education (“NSHE”). I understand and hereby acknowledge that my participation in the Nevada Spirit Team Try-outs is wholly voluntary. In consideration of being allowed to participate in the Nevada Spirit Team Try-outs at the University of Nevada, Reno I hereby agree as follows:
1. I understand the nature of the Nevada Spirit Team Try-outs I may participate in (i.e.: cheering, dancing, stunting, tumbling, jumping, conditioning) may require mental judgment and a high degree of physical fitness, agility, and dexterity, and that this may include strenuous exercise that requires physical fitness, strength, and stamina.
2. I understand that the Nevada Spirit Team Try-outs described herein involve the risk of injury or death and the risk of damage to or loss of property. I understand that these activities involve serious dangers and inherent risks, including risks associated with travel to and from such activities in Nevada and California, as well as travel to and in mountainous regions and other remote places. I understand, accept, and voluntarily assume these risks.
3. I understand that neither the NSHE nor the University of Nevada, Reno, will provide medical or health insurance coverage to me during any aspect of my participation in the Nevada Spirit Team Try-outs at the University of Nevada, Reno. I hereby represent and warrant that I am and will be covered throughout the activity by a policy of comprehensive health and accident insurance that provides coverage for injuries I may sustain in the course of my participation in the activity. I understand I may be required to show proof of insurance coverage prior to my participation in the activity.
4. I, individually, and on behalf of my heirs, successors, assigns and personal representatives, hereby agree to indemnify, defend, and hold harmless the NSHE and the University of Nevada, Reno, and their employees, agents, and representatives, from any and all liability whatsoever for any and all
damages, losses, or injuries (including death) I sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, judgments, expenses and costs, including attorney’s fees, which arise out of, result from, occur during, or are connected in any manner with my participation in the activity.
5. To the extent authorized by law, I, individually, and on behalf of my heirs, successors, assigns and personal representatives, hereby release and forever discharge the NSHE and the University of Nevada, Reno, and their employees, agents, and representatives, from any and all liability, loss, damage or expense, including attorney’s fees, that they or any of them incur or sustain as a result of any claims, demands, actions, causes of action, damages, judgments, costs or expenses, including attorney’s fees, which arise out of, occur during, or are in any way connected with my participation in the activity.
6. To the extent that I, individually, or my heirs, successors, assigns, or personal representatives bring a claim of any kind whatsoever against the NSHE and/or the University of Nevada, Reno, and/or their employees, agents, and representatives, I agree that this Waiver, Release and Indemnification Agreement is to be construed under the laws of the State of Nevada, including the provisions of Nevada Revised Statutes Chapter 41; and that if any portion hereof is held invalid, the balance hereof shall, notwithstanding, continue in full legal force and effect. In signing this document, I hereby acknowledge that I have read this entire document, that I understand its terms, that by signing it I am giving up substantial legal rights I might otherwise have, and that I have signed it knowingly and voluntarily.
Certificate of Eligibility Nevada Spirit Program
In order to be eligible for participation in the Nevada Spirit Program; Cheer, Stunt, Pom or Mascot, the student must be accepted to the University of Nevada.
If he/she is already a student at Nevada, then he/she must have a cumulative and semester 2.0 GPA.
The student must maintain a cumulative 2.0 GPA per semester and maintain full time enrollment (12 credit hours) at Nevada if selected as part of the Nevada Spirit Program (Cheer, Stunt, Pom, Tumble or Mascot). All of the above requirements must be met when grades come out for the 2025 spring semester, if the applicant makes the team.
If for any reason the student falls below full time or does not maintain a cumulative/semester GPA of 2.0 while on the team, they will be removed from the Spirit Program. If the student does not fulfill any of the responsibilities of the Spirit Program, the student will be suspended or removed from the program. There will be no replacement grades available.
By signing this paper, I am authorizing the administration and staff of the University of Nevada to confirm my academic eligibility. If the student is successfully chosen, as a member of the Nevada Spirit Program, this form will be held on file all year, for academic eligibility checks.
I acknowledge my understanding of the grade eligibility of this document and I understand that school and obtaining a degree from the University of Nevada is the number one priority.
RELEASE OF ACADEMIC, EMPLOYMENT and FINANCIAL INFORMATION
By signing this form, I give my consent to disclose the documents below to authorized representatives of the University of Nevada, the Mountain West Conference, the NCAA, my parents or legal guardians, my coaches, and various outside organizations and associations for the purpose of my qualification for academic recognition or honorary awards and to determine my academic and athletic eligibility. I authorize University of Nevada faculty members to release my transcript and grade information to University of Nevada Athletic Academic Advisors. I understand the Family Educational Rights and Privacy Act of 1974 protects these records and they may not be disclosed without my consent.
These records include:
1. Any transcript from my high school, the University of Nevada or any junior college or any four-year institution have attended;
2. Pre-College test scores and any information relating to eligibility for non-standard testing;
3. Graduation status;
4. Race and Gender identification;
5. Records concerning my financial aid
6. Employment earnings and payroll documents
7. Any other documents or information, including financial information pertaining to my NCAA eligibility. This includes car loan information, insurance information, bank statements, home loans, and rental leases.
I understand these records will be disclosed only to determine my eligibility for intercollegiate athletics, determine my eligibility for athletically related financial aid, determine my eligibility for academic recognition or honorary awards, evaluate my school’s academic success, and assist the NCAA in compiling a summary of institutional information that will be publicly released.
